ABSTRACT

Objective:To evaluate the clinical significance of our self-designed three-column scoring system for postoperative X-ray stability of intertrochanteric fracture after intramedullary nailing.Methods:A retrospective study was conducted of the 378 patients with intertrochanteric fracture who had been treated between January 2015 and June 2019 at Department of Orthopaedics, Linyi People's Hospital by internal fixation with proximal femoral nail antirotation (PFNA). They were 161 males and 217 females, aged from 60 to 97 years (average, 72.5 years). By the AO classification, there were 109 cases of type 31-A1, 188 cases of type 31-A2 and 81 cases of type 31-A3. Anteroposterior and lateral X-ray examinations of the hip were performed immediately after internal fixation to evaluate the fracture stability using our self-designed three-column scoring system by which the medial column is given 4 points, middle column 2 points and lateral column 2 points. A single column scoring full points is rated as stable, 3-column stability as excellent, 2-column stability as good, one-column stability as fair, and 3-column unstability as poor. Rehabilitation programs were carried out according to the results of stability evaluation: full weight bearing at an early stage was indicated for excellent patients, partial weight bearing at an early stage for good patients and weight bearing at an early stage contraindicated for fair or poor patients. Fracture union time and failure of internal fixation were recorded. The relationship between internal fixation failure and our three-column scoring system was calculated.Results:The 378 patients were followed up for 6 to 24 months (mean, 10.4 months). Of them, 365 obtained fracture union after an average time of 4.3 months (from 3 to 7 months). Internal fixation failure occurred in 13 patients, giving a failure rate of 3.4%(13/378).Of the 129 excellent patients by our three-column scoring system for post-operative X-ray stability of intertrochanteric fracture, none failed in internal fixation; of the 193 good patients, 4 failed; of the 56 fair patients, 9 failed. Internal fixation failure was closely related to our three-column scoring system for postoperative X-ray stability of intertrochanteric fracture ( r=-0.986, P=0.006), as well as to the column stability ( r=-1.000, P=0.033). Conclusion:Our self-designed three-column scoring system for postoperative X-ray stability of intertrochanteric fracture after intramedullary nailing can accurately reflect the fracture stability so that it can be used to guide rehabilitation programs for the patients and judge their prognosis.

ABSTRACT

Objective@#To analyzed demographic and clinical characteristics in hospitalized patients with osteoporotic vertebral compression fractures(OVCFs)and to provide certain evidence for prevention and treatment of the disease.@*Methods@#Retrospectively analyzedthe clinical data of 2 028 patients with OVCF from January 2009 to December 2018 in the Spine Department of Qilu Hospital of Shandong University. The trend of composition ratio and average age of OVCF patients in the past ten years were observed, the differences of the demographic characteristics and clinical characteristics between different genders as well as the correlation between the number of fractured vertebral bodies of OVCF and various influencing factors were analyzed, and the characteristics of OVCF inpatients were summarized.@*Results@#Of 2 028 patients, 374 were males and 1654 were females.The average age was 70.50±9.73 years old, and the male is 73.29±10.23 and the female is 69.87±9.46. A significant increase in the constituent ratio of OVCF between 2009 and 2018 (6.85% in 2009 vs. 13.24% in 2013) and the mean age of OVCF patients increased from 67.54±9.92 years in 2009 to 70.96±9.63 years in 2018were identified.There were significant differences in age distribution of OVCFs in different gender. When the age was less than 70 years old,the fracture ratio of female was higher than that of males, especially between 60 and 70 years old (38.63%∶17.91%); However, when the age was more than 70 years, the fracture ratio of male was higher than that of female especially after 80 years old(29.15%:12.70%). There were significant differences in occupation distribution of OVCFs indifferent gender. Peasants (47.94%), professionals (18.32%) and workers (14.63%) were the top three in female while peasants (35.56%), workers (26.47%) and professionals(20.05%) in male. Falling was the main injury cause of OVCF and there were significant differences in the distribution of causes of OVCF injuries between different genders. The proportion of traffic accident injuries infemaleswas higher than that in maleswhile the proportion of sprains inmaleswas higher than that in females.Moreover,there was a statistically significant difference in the distribution of fracture vertebral bodies between patients of different ages, with or without fracture history and smoking history and different ages of menopause.Among them, the number of fractured vertebral bodies was positively correlated with age(r=0.79, P<0.05).The composition ratio of single vertebral fracture of patients without fracture history, menopausal ageunder 50 and withoutsmoking history was higher than that with fracture history, menopausal age over 50 and with smoking history. On the contrary,the composition ratio of two or more vertebral fractures in patients without fracture history and menopausal agebelow 50 was lower than thatwith fracture history and menopausal ageover 50. Particularly, the ratio of three or more vertebral fractures with smoking history was higher than thatwithout smoking history.The number of patients who adopted bone mineral density(BMD) test and bone turnover maker test was 118 and 106 before 2014 while the number was 568 and 837 after 2014 and the differences were statistically significant.@*Conclusion@#The proportion of OVCFs in spine diseases and average age of patients in the period of 2009-2018 was increasing gradually. Distribution of the age,occupation and cause of injury were related to the gender, but there was no correlation between distribution of season, fracture segment and comorbidities and gender. The age, previous fracture history, female menopausal age and smoking history were correlated with the number of fractured vertebral bodies while drinking history was not related.

ABSTRACT

Objective To study the accuracy of the anteversion of femoral prosthesis in hemiarthro-plasty for treating femoral intertrochanteric fracture in the elderly.Methods Twenty femoral neck antever-sions of specimens were measured, which were control group(A).The femoral specimens were made into the model of femoral intertrochanteric fracture, which were installed femoral prosthesis according to minor trochanter(B),identifying the max diameter of cavum medulla(C),ocular estimating femoral condylar line ( D) ,combination of identifying the max diameter of cavum medulla and ocular estimating femoral condylar line( E) respectively.All the anteversion of femoral prosthesis were measured.Results The anteversion were:A group(8.98 ±4.31)°,B group (9.12 ±4.87)°,C group (9.57 ±6.49)°,D group (13.18 ± 5.83)°,E group (10.37 ±4.56)°respectively.The difference among the five groups was statistically sig-nificant.There was difference between D and A, there were no differences between B and A, C and A, E and A respectively.Conclusions In hemiarthroplasty for treating femoral intertrochanteric fracture, the an-teversion of femoral prosthesis is according to minor trochanter possibly; if the minor trochanter can not be got reduction, it is a good option according to combination of identifying the max diameter of cavum medulla and ocular estimating femoral condylar line to make sure the accuracy of the anteversion of femoral prosthe-sis.

ABSTRACT

Objective To study the interventional effects of Candesartan Cilexetil on vascular endothelial function and expres-sions of ERK/CREB signaling pathway in rats with acute myocardial infarction.Methods 30 rats were randomized into CAN group,AMI group and Sham group.Two weeks after the treatment rats were sacrificed and blood pressure,levels of blood AngⅡ, NO and NOS,endothelial vasomotor effects of isolated thoracic aorta strips,myocardial infarctual area and the expressions of ERK mRNA and CREB mRNA in infarctual myocardium were explored.Results Blood AngⅡ levels were distinctly higher in AMI group when compared with the Sham group,while the NO and NOS level were much lower.accompanied with EDDs decreased seri-ously,and higher mRNA expressions of ERK1 and CREB in infarctual myocardium.All of the above differences were significant. After the treatment of Candesartan Cilexetil,levels of serum NO and activities of NOS were obviously higher and almost reached the similar levels to those in Sham group,additionally endothelium-dependent diastole in isolated aortic strips improved greatly.Mean-while Candesartan Cilexetil can not only increase the plasma AngⅡ,but also decrease the size of myocardial infarction and the mR-NA expressions of ERK1 and CREB in infarctual myocardium significantly.However,blood pressure in all groups was not affected. Conclusion Candesartan Cilexetil can greatly improve the disordered endothelial function developing post-AMI,decrease the size of myocardial infarction and down-regulate the mRNA expressions of ERK and CREB in myocardium of infarctual zone.And all of the above protective effects of Candesartan Cilexetil were independent on blood pressure lowering.

ABSTRACT

Objective To investigate whether the implanted spike wave electrical stimulation (ES) can improve survival of mesenchymal stem cells (MSCs) after transplantation in spinal cord injury (SCI) rats. MethodsIn the study,60 male SD rats were used to prepare the SCI induced by a digital weight drop apparatus.The MSCs from each rat' s femurs were collected and then labeled by Bromodeoxyuridine (Brdu) seven days after SCI.The rats were randomly assigned into four groups,ie,group A (receiving a transplantation of MSCs only),group B (receiving electro-stimulant therapy only),group C ( receiving a transplation of MSCs and electro-stimulant therapy) and the control group ( receiving a single injection of phosphate buffer solution,PBS).Functional status was assessed regularly before and after SCI by using the Basso-Beattie-Bresnahan (BBB) locomotor rating score and somatosensory evoked potential (SEP).The recovery of spinal cord was checked by diffusion tensor imaging (DTI) and the survivorship of the labeled MSCs was observed with immunohistochemical method. Results BBB score of group C demonstrated significant difference in comparison with that of the other three groups ( P < 0.05 )and was significantly higher than that of the control group (P <0.01 ) four weeks after surgery,which indicated an obvious functional improvement of the extremity mobility in group C.At the 8th week postoperatively,SEP study displayed significant difference in group C from the other three groups in terms of latency and wave amplitude (P <0.05 ),indicating the fast neurofunctional recovery of the spinal cord in group C.MR images of the spinal cords by DTI showed the same outcome.Immunohistochemistry confirmed a large number of the labeled MSCs positive cells in the lesion site of group C.ConclusionTheimplanted spike wave ES promotes the bioactivity and survival of MSCs and improves the therapeutic effect in combination with MSCs transplantation.

ABSTRACT

ObjectiveTo study the expressions of p75NTR,Bax and Bcl-2 and cell apoptosis in rat cortical neurons following mechanical injury and discuss the mechanism and mutual action way during the apoptosis of rat neurons after mechanical injury.Methods Cortical neuron cultures were prepared from the brain tissues of day 17 rat embryos and were exposed to mechanical injury seven days after seeding.After the traumatic neuron injury models were created,the apoptosis ratio of neurons was tested at several time points.The models were divided into the minor,moderate,severe injury groups according to the injury severity and the control group.The expressions of Bax and Bcl-2 in each group were detected by immunohistochemistry method and that of p75NTR by Western-blot.Combining with cellular apoptosis ratio in each group shown after FCM analysis,the correlation between the expressions of p75NTR,Bax and Bcl-2 and the apoptosis in rat neurons after mechanical injury could be analyzed.ResultsThe apoptosis ratio of the neurons in all the injury groups was obviously higher than that in the control group,with significantly higher apoptosis ratio of the neurons in the severe injury group than the minor and moderate injury groups (P <0.05).P75NTR,Bax and Bcl-2 were all expressed in all the injury groups,with statistical differences between groups.The expression of Bax in the severe injury group was significantly higher than that in the minor and moderate injury groups(P < 0.05 ). Conclusions p75NTR expression and Bax/Bcl-2 ratio are closely correlated with neuron apoptosis.The early expression of p75NTR may be one of mechnisms for neuronal apoptosis after neuron injury,when Bax and Bcl-2 may be involved.

ABSTRACT

[Objective]To research the influence of spinal cord blood flow(SCBF) and the evoked potential(EP) by the extramarrow impacting and to explore the correlation and regularity between the spinal cord blood flow and evoked potential after spinal cord injury.[Method]Normal 120 SD rats were randomized into 4 groups,with 30 in each.Group A was the control group.In groups B,C,D,impacts of 10,15,20N were given to lamina respectively.The blood flow of impacting position and evoked potential were observed at different time points including 0 min,1 h,6 h,1 d,6 d,12 d,24 d.[Result]The blood flow of spinal cord represents a dimning trend with the impacting load raising.The blood flow of group A,had no marked change,the SEP and MEP also.The blood flow of group B represented a temporaly lowing,then recovered normal,the SEP and MEP also.The blood flow of group C and D represented a continuous lowing,then slowly raising.The group C recovered normal at 24d,the group D still lower than normal.[Conclusion]The impact load to lamina can influence the spinal cord function through altering the spinal cord blood flow.There is evident correlation between the SCBF and EP.

ABSTRACT

[Objective]To investigate the changes and correlation of bone collagen and biomechanical property in osteoporosis rats.[Method]Normal 30 nullimating female 7 month SD rats were divided randomly to two groups,each included 15.The sham group was unovariectomized,and the OVX group was ovariectomized.To execute the rats after 12 weeks,the author get the 5th lumbar vertebra,and then detected the biomechanical property and HYP.The author observed the collagen morphology of the 4th lumbar vertebra using the microscope after the masson triad color dyeing.[Result]Twelve weeks later,HYP in the sham group was(0.197?0.035)mg/g.The bone collagen was regular,compact,and closed succession.HYP in the OVX group was(0.182?0.022)mg/g.The bone collagen was incompact,thin,collapse and poor succession.There was significant change of the HYP in two groups(P

ABSTRACT

Objective To prove the prophylactic function of absorbable gelatin sponge and bioprotein glue for prevention of epidural and nerve root scar adhesions after laminectomy. Methods We excised the L6 vertebral plates of 36 adult New Zealand male rabbits,to make vertebral plate excision model and to find nerve root. Then divided them into 3 groups based on different materials:gelatin sponge and bioproten glue (group A),gelatin sponge (group B) and saline solution (group C). The morphological changes of the scars were observed grossly and histologically at 2 and 8 weeks after operation. We also measured the orbit of the nerve root. ABSTRACT

Objective To study the influences of impact of different loads to lamina on spinal cord with a closed spinal cord injury model and to explore the mechanisms of blood vessels in spinal cord injury. Methods 120 normal male SD rats were randomized into 4 groups, with 30 in each. Group A served as the control group which received no impact. In Groups B, C and D,impacts of 10, 15 and 20 N were given to lamina respectively. The blood flow in the spinal cord central gray matter and the dorsal white matter, the blood pressure and the heart rate were observed at different experimental time points. Results No obvious change was observed in the blood pressure and heart rate in every group at different time points. The spinal cord blood flow (SCBF) in Groups C and D varied in a regular pattern. Conclusions The impact load to lamina can affect the spinal cord mainly by altering the spinal cord microcirculation. The influence is the most obvious on the blood flow of microcirculation of the gray matter.

ABSTRACT

<p><b>OBJECTIVE</b>To observe reversal of ventricular remodeling by the protein kinase C inhibitor Scutellarein in spontaneously hypertensive rats (SHRs).</p><p><b>METHODS</b>Twelve SHRs were randomly divided into two groups. Scutellarein and saline (10 mg x kg(-1) x d(-1)) were given by intraperitoneal injection to two groups of rats separately. Systolic blood pressure (SBP) and ventricular weight index (LVW/BW, RVW/BW) were measured. A polarization microscope and an image analyzer system (IAS) were used to observe changes in cardiovascular structure and to count the content of cardiac muscle interstitial collagen.</p><p><b>RESULTS</b>The pathologic changes in the left ventricle in the Scutellarein group rats (SHR(D)) improved to varying degrees, including hypertrophy of the cardiac muscle and collagen volume fraction.</p><p><b>CONCLUSION</b>Scutellarein can reverse ventricular remodeling, improve myocardial stiffness and protect heart cardiac muscle.</p>

ABSTRACT

AIM: To study the effects of oxidized high-density lipoprotein (oxHDL) on the expression of monocyte chemoattractant protein-1(MCP-1) and intercellular adhesion molecule-1(ICAM-1) and intracellular free calcium concentration ([Ca2+]i) level in cultured human umbilical venous endothelial cells(HUVECs). METHODS: The MCP-1 protein content in the medium of conditioned HUVEC was measured by ELISA, and the ICAM-1 on HUVECs was detected by indirect immunofluorescence, and [Ca2+]i was determined by Fluo-3/AM, the injury of cells was observed by scanning electron microscopy (SEM).RESULTS: oxHDL could induce the expression of MCP-1 and ICAM-1 in HUVECs. In oxHDL group (HUVECs were incubated with 100 mg protein/L oxHDL for 24 h), the levels of MCP-1, ICAM-1 and [Ca2+]i increased by 160%, 60% and 70% respectively compared with the control group (P<0.01). When HUVECs were incubated with 300 mg protein/L oxHDL for 24 h, cells were injured obviously. CONCLUSION: By inducing the expression of ICAM-1 and MCP-1 in endothelial cells, oxHDL may promote monocyte-endothelium adhesion and monocyte migration to intima, it may promote atherosclerosis as oxidized low-density lipoprotein (oxLDL).

ABSTRACT

Objective To find a new way to promote the regeneration of injured peripheral nerve after surgical repair, and to interpret its application and advantages. Methods Eighty adult male Wistar rats with sciatic transplantation were divided into group A(control group) and group B(exponential curve electric stimulus group). The effect of pace, hair and unfold claw reflex were observed, and the sciatic nerve function index(SFI), nerve conduction velocity, wet weight of triceps surae were measured at different time. The curative effects of two groups were compared statistically. Results The group of exponential curve electric stimulus had positive effect on the recovery of nerve conduction velocity, the slower of the denervated disuse of triceps surae, the recovery of SFI, the acceleration of distal nerve degeneration, at last promote the injured nerve regeneration and the good recovery of function. The group of exponential curve electric stimulus is of significant difference statistically with the control group. Conclusion Exponential curve electric stimulus can promote the injured nerve regeneration.

ABSTRACT

AIM: To study the discrepancy and it's significance of ATP binding cassette transporter(ABC1) and CD36 mRNA expression while activate nuclear receptor RRAR?,LXR?,RXR? in THP-1 cells. METHODS: Cultured THP-1 cells were induced to become macrophage by PMA at concentration of 40 ?g/L for 48 hours. Troglitazone, 22(R)-hydrocholesterol and 9-cis-retinoid acid(9-CRA),activators for PPAR?,LXR? and RXR? respectively, were incubated with the macrophage for 24 hours. Total RNA of these cells were abstracted and reverse transcriptional polymerase chain reaction were performed to inspect the expression of ABC1 and CD36 mRNA in the cells. RESULTS: Expression of ABC1 mRNA increased in the presence of troglitazone, 22(R)-HC and 9-CRA. CD36 mRNA also increased while PPAR? activated. Activation of LXR? showed almost no effect on CD36 mRNA expression, and expression of CD36 mRNA was decreased by 9-CRA.CONCLUSION: The discrepancies existing in ABC1 and CD36 mRNA expression while activate nuclear receptors PPAR?,LXR?,RXR? in THP-1 cells implyed that LXR?,RXR? were better targets to deal with to improve cholesterol reverse transportation.

ABSTRACT

AIM: To investigate the effect of tea-polyphe nols (TP) on the activation of NF-?B and the expression of TGF-? 1 mRNA in TH P -1 cells (a human acute monocytic leukemia cell line). METHODS: TH P-1 cells were incubated with the different concentrations of TP, VLDL, LDL or o x-LDL. In the THP-1 cellls, the nuclear malposition rate of NF-?B was detected with immunohistochemistry technique, the positive index of the TGF-? 1 mRNA ex p ression was detected by hybridization in situ, and accumulation of total cholest erol (TC) in cells incubated with 0 4-40 ?g/L TP was determined with oxidase a ssay. RESULTS: The nuclear malposition rate of NF-?B, the posit ive index of the TGF-? 1 mRNA expression and TC in THP-1 cells incubated with 0 4-40 ?g/L of TP were lower than those with 0 ?g/L of TP in TP-V group, TP-L group and TP-O ( P 0.05). CONCLUSION: TP inhibited the activation of NF-?B, the expressio n of TGF-? 1 mRNA and the foam cell formation in the mono-macrophage.

The LDH was higher ,and cell viability was lower in grou p SMT, group NS 398 and group SB than those in group IPC. CONCLUSIONS: In late p hase of preconditioning ,the p38MAPK activity and expression of iNOS and COX 2 increase significantly in rat cardiomyocytes, activited p38MAPK mediates iNOS ,then promotes COX 2 expression.
